Heart Screening Clinic Timetables
Dear Parents/Students,
Please find attached the timetables for our heart screening clinic that will be taking place in room 1/19 (school library) on Monday and Tuesday this upcoming week. We would advise your son to check in at the room 10 minutes prior to his scheduled time.
Patient questionnaires were handed out last Thursday and Friday to those who were in school and these need to be fully filled out and brought to the appointment also. For those that were absent or did not get the questionnaire, it can still be filled out at the appointment. The key piece of information to be filled out on the form is your GP Name and Address.
Students must be in full uniform, however we request that they bring/wear a t-shirt to their appointment to make it easier to conduct the tests. If you have any questions in the meantime please do not hesitate to contact me (smg@naascbs.ie) and if there is any reason your son cannot make his scheduled appointment please let me know and we will do our best to accommodate changes.
